Congressional district maps
Congressional district maps display death rates and locations of health care facilities.
There are three series of congressional district maps:
- Heart Disease Mortality and Federally Qualified Health Centers (FQHCs)
- Stroke Mortality and Federally Qualified Health Centers (FQHCs)
- Stroke Mortality and Stroke Centers
To view and download a congressional district map:
- Visit the Map Series section of this page.
- On this page, select the series of interest to you.
- Then select a state from the drop down menu.
- You will be directed to CDC Stacks where you can select and download the congressional district map(s) of interest to you.
