This map showcases the disease burden of the counties located in the Mississippi Delta region and one of the many social drivers that adversely impact the residents in this region's ability to achieve a healthy lifestyle. It also expresses the need for partnerships, resources, and social needs that address the gaps in disproportionately impacted areas due to decreased access to healthcare services, financial instability, and lack of a built environment.

County-level data on high blood pressure, high cholesterol, and housing burden costs were displayed using tertiles. Bivariate maps were created, making it easy to identify counties in the highest tertile for all three variables.

The Mississippi Delta Health Collaborative (MDHC) Program and National Heart Disease and Stroke Prevention Program (HDSPP) will utilize the data presented in this map to meet the current needs of this population, delve deeper into the other social drivers of health, and tailor healthcare and social resources to promote healthy behaviors while increasing the quality of life.
