The Living Healthy in Washington County Resource and Referral Initiative is intended to build connections between community resources and healthcare partners to facilitate prevention counseling for patients at risk for obesity and tobacco related diseases, including heart disease, stroke, cancer, and diabetes. Through this initiative, healthcare providers are able to utilize community resource information when referring their patients to be more active, eat healthier, or reduce tobacco used and exposure. Mapping the resources will allow providers and patients to quickly identify the best resources for their needs.
