Key points

Through the CDC 1422 grant, the Hawaii Primary Care Association, the Hawaii Public Health Institute, and the Hawaii State Department of Health are working to identify adults with undiagnosed high blood pressure in the priority population and to increase access to healthy foods, through the Choose Healthy Now program, in nine target communities throughout the state. This map shows both the progress Hawaii has achieved in increasing access to healthy foods and the areas where additional Choose Healthy Now locations are needed to support adults in the target communities with newly diagnosed high blood pressure.

This map shows self-reported crude-prevalence of hypertension among Native Hawaiian, other Pacific Islander, Filipino, and/or low-income adults, Hawaii’s priority population for the 1422 Cooperative Agreement, from the 2013 and 2015 Behavioral Risk Factor Surveillance System. Prevalence of high blood pressure was classified in quantiles, by community. Communities served by a federally qualified health center (FQHC) were highlighted with a thick, blue border, and Choose Healthy Now locations were symbolized with a white circle with a green border to mimic the Choose Healthy Now logo. Rural communities tended to have higher rates of hypertension than urban communities.

Data sources

Hawaii’s Priority Population for the CDC 1422 grant, Native Hawaiians, Pacific Islanders, Filipinos, and/or low income adults, experience high rates of high blood pressure, especially in rural communities in Hawaii. Ensuring access to nutritious foods is one way to help people make lifestyle changes to reduce their blood pressure. The Hawaii State Department of Health and partners are working to increase access to healthy foods in target communities throughout the state by increasing the number of retailers in their community that participate in the Choose Healthy Now program.

This map is being used by the Hawaii Department of Health and partners to both track the progress Hawaii has achieved in increasing access to healthy foods and to identify the areas where additional Choose Healthy Now locations are needed to support adults in the target communities with newly diagnosed high blood pressure.
