Despite achieving the statewide Healthy People 2020 (HP2020) colorectal cancer screening prevalence of 70.5% in 2015, large disparities exist at a subcounty community level in Hawaii, highlighting the need to target communities experiencing disparities with enhanced resource allocation and intervention.

This map displays community-level 2015 colorectal cancer screening rates among adults 50 to 75 years for the state of Hawaii. In 2015, Hawaii met the Healthy People 2020 statewide target for colorectal cancer screening prevalence (70.5%) and set the Hawaii State Cancer Plan (2016–2020) target to be 80.0%. Despite this success, significant disparities exist by community, particularly on the islands of Moloka‘i and Kaua‘i where screening prevalence is as low as 43.4% and 59.4%, respectively.

Data sources

Hawaii’s Priority Population for the CDC 1422 grant, Native Hawaiians, Pacific Islanders, Filipinos, and/or low income adults, experience high rates of high blood pressure, especially in rural communities in Hawaii. Ensuring access to nutritious foods is one way to help people make lifestyle changes to reduce their blood pressure. The Hawaii State Department of Health and partners are working to increase access to healthy foods in target communities throughout the state by increasing the number of retailers in their community that participate in the Choose Healthy Now program.

This map is being used by the Hawaii Department of Health and partners to both track the progress Hawaii has achieved in increasing access to healthy foods and to identify the areas where additional Choose Healthy Now locations are needed to support adults in the target communities with newly diagnosed high blood pressure.

Crude prevalence of self-reported high blood pressure among Native Hawaiian, Pacific Islander, Filipino, and/or low income adults were classified in quantiles, by community. Communities served by an FQHC were highlighted with a thick blue border, and Choose Healthy Now locations were symbolized with a white circle with a green border to mimic the Choose Healthy Now logo.
