Data sources

This map helps to identify areas in need of additional Choose Healthy Now programmatic efforts to support healthy eating behaviors among patients enrolled in DPPs and SMBP programs.

This map will be used to guide the efforts of the Department of Health’s healthy vending program, Choose Healthy Now, in identifying areas of need. One area of focus is to support healthy food environments around DPPs and SMBP programs in order to reinforce dietary behaviors promoted in those programs.
