PDF documents available for download are for informational purposes only and should not be used to collect or submit data. No paper copies of the survey will be accepted during survey administration years.
During survey administration years, CDC contractor Battelle contacts hospitals to provide links to hospital surveys. Visit the Methods page for more information. Hospitals do not need to contact CDC or Battelle unless they received the survey link and have questions.
