PreventT2 Curriculum and Handouts

What to know

The PreventT2 curriculum is based on the original 2002 Diabetes Prevention Program (DPP) trial and follow-up studies. It promotes modest weight loss (5%–7%) and increased physical activity through a 12-month lifestyle change program. It also reflects new literature on self-efficacy, physical activity, and diet. On this page, learn about curriculum requirements and download a CDC-developed curriculum in English.

Access the curriculum and handouts

There are 26 modules in the PreventT2 curriculum. To achieve CDC recognition for your National DPP lifestyle change program, you must complete at least 22 of these modules. These materials are also available in Spanish. Please see the chart below for details.

First 6 months of program

You must present all 16 of these modules within the first 6 months of your lifestyle change program. Introduction to the Program must come first. Otherwise, there is no required order.

However, we recommend that you present the next six modules in the order listed because they provide a foundation for the rest of the curriculum. We also recommend presenting Stay Motivated to Prevent T2 at the 6-month mark.

You must present at least six of these 10 modules during the last 6 months of your program. The Prevent T2—for Life! module must come last. Otherwise, there is no required order.

Supplemental handouts

Lifestyle Coach Log [PDF-857KB]
Use the lifestyle coach log to record participants' data for each session.

Fitness Log [PDF-438KB]
Encourage participants to use their fitness log each day to track their minutes of activity (starting after the Track Your Activity module). The activity should be of at least a moderate pace. Participants show their fitness log at the start of each session.

Food Log [PDF-2.1MB]
Participants are encouraged to use their food log to track their eating (starting after the Track Your Food module).

Weight Log [PDF-731KB]
At each session, weigh participants in private and let them know how much they weigh. Participants may record this number in their weight log.

Action Plan Journal [PDF-4.3MB]
Participants are encouraged to make a new action plan during each session and to try it at home.
