2Excludes patients who were never started on TB therapy; patients who died within one year of initiating therapy; patients with an initial isolate resistant to rifampin; patients with bone and joint disease, meningeal disease, or disease of the central nervous system; or patients <15 years of age with miliary disease, positive blood culture, or a positive nucleic acid amplification test on a blood specimen. Patients who moved out of the country within one year of initiating therapy were also excluded.
