Welcome to the National Health Care Surveys Registry!

The National Health Care Surveys Registry is one way to meet the public health reporting requirement of the Centers for Medicare and Medicaid Services (CMS) Electronic Health Records (EHR) Incentive Programs—

You can join the registry if you are an eligible—

  • Clinician
  • Professional
  • Hospital
  • Critical access hospital

Through the registry, you can record your intent to submit EHR data if the National Center for Health Statistics (NCHS) invites you to participate in one of the National Health Care Surveys.

EHR Incentive Program requirement

Participating in CMS's PI program requires healthcare providers and facilities to take part in public health reporting using Certified EHR Technology (CEHRT). This type of reporting involves "active engagement" with a public health agency or approved organization (entity) by—

  • Registering intent to submit data
  • Testing and validating those data with the public health entity
  • Exchanging production data

If you participate in an EHR Incentive Program, registering your intent to submit data through the National Health Care Surveys Registry Portal immediately puts you in active engagement with NCHS.

If invited to participate in one of the National Health Care Surveys, you must move to the next stages to maintain active engagement status—

  • Registration of Intent or Testing and Validation (Option 1)
  • Validated Data Production (Option 2)

Eligible hospitals, critical access hospitals, and clinicians may remain in active engagement Option 1 for only on EHR reporting period.

The National Health Care Survey Registry Portal is a web-based application you can access on a computer, tablet, or smart phone. We recommend using Google Chrome, Microsoft Edge, or Mozilla Firefox as your web browser for a better user experience. The National Health Care Survey Registry Portal is not compatible with Internet Explorer.
