Next Steps

  • You should receive an email in the next 15 minutes with your receipt. That email will come from DGMQAdminSupport@cdc.gov and you may need to check your spam folder.
  • Please retain a copy of your CDC Dog Import Form receipt. You must present the receipt to the airline if the dog is traveling by air and U.S. Customs and Border Protection on arrival in the U.S.
