Immunization information systems (IIS) are confidential, population-based, computerized databases that record all immunization doses administered by participating providers to persons residing within a given geopolitical area. This information details how to access immunization records for vaccine recipients who received a vaccine in Wisconsin.
General Information
Wisconsin's IIS is the Wisconsin Immunization Registry (WIR), and it includes immunization records for vaccine recipients of all ages. Visit this webpage to learn more about the IIS.
The jurisdiction is authorized by law to operate WIR. For more information, please see Wisconsin Statutes § 250.04 as well as § 252.02(1) and § 252.04(1).
Child Consent to Participate
For children, WIR requires implicit consent with opt-out by operational policy. Children are included in the IIS unless their parent(s)/guardian(s) choose to opt them out.
Adult Consent to Participate
For adults, WIR requires implicit consent with opt-out by operational policy. Adults are included in the IIS unless they choose to opt out.
Provider Reporting
Wisconsin does have a mandate for vaccination providers to report immunizations to WIR. The reporting mandate:
- Applies only to certain providers and
- Applies only to specific immunizations.
