What to know

Immunization information systems (IIS) are confidential, population-based, computerized databases that record all immunization doses administered by participating providers to persons residing within a given geopolitical area. This information details how to access immunization records for vaccine recipients who received a vaccine in Utah.

General Information

Utah's IIS is the Utah Statewide Immunization Information System (USIIS), and it includes immunization records for vaccine recipients of all ages. Visit this webpage to learn more about the IIS.

Child Consent to Participate

For children, USIIS requires implicit consent with opt-out by law. Children are included in the IIS unless their parent(s)/guardian(s) choose to opt them out.

For adults, USIIS requires implicit consent with opt-out by law. Adults are included in the IIS unless they choose to opt out.

Provider Reporting

Utah does have a mandate for vaccination providers to report immunizations to USIIS. The operational reporting mandate:

  • Applies only to certain providers.

Data Sharing

Utah does have a policy that regulates sharing data from USIIS with other public health entities. The data sharing policy:
