Immunization information systems (IIS) are confidential, population-based, computerized databases that record all immunization doses administered by participating providers to persons residing within a given geopolitical area. This information details how to access immunization records for vaccine recipients who received a vaccine in Philadelphia.

General Information

Philadelphia's IIS is called PhilaVax, and it includes immunization records for vaccine recipients of all ages. Visit this webpage to learn more about the IIS.

Child Consent to Participate

For children, PhilaVax requires implicit consent with opt-out by law. Children are included in the IIS unless their parent(s)/guardian(s) choose to opt them out.

Adult Consent to Participate

For adults, PhilaVax requires implicit consent with opt-out by law. Adults are included in the IIS unless they choose to opt out.

Provider Reporting

Philadelphia does have a mandate for vaccination providers to report immunizations to PhilaVax. The reporting mandate:

Data Sharing
