What to know

Immunization information systems (IIS) are confidential, population-based, computerized databases that record all immunization doses administered by participating providers to persons residing within a given geopolitical area. This information details how to access immunization records for vaccine recipients who received a vaccine in Oregon.

General Information

Oregon's IIS is the ALERT Immunization Information System, and it includes immunization records for vaccine recipients of all ages. Visit this webpage to learn more about the IIS.

Child Consent to Participate

For children, ALERT requires implicit consent with opt-out by law. Children are included in the IIS unless their parent(s)/guardian(s) choose to opt them out.

Adult Consent to Participate

For adults, ALERT requires implicit consent with opt-out by law. Adults are included in the IIS unless they choose to opt out.

Provider Reporting

Oregon does have a mandate for vaccination providers to report immunizations to ALERT. The reporting mandate:

  • Applies only to certain providers and
  • Applies only to specific immunizations.

Data Sharing

Oregon does have a policy that regulates sharing data from ALERT with other public health entities. The data sharing policy:

  • Requires a data use/sharing agreement or memorandum of understanding and
  • Requires protecting the data's confidentiality.

The jurisdiction’s child consent policy addresses parents/guardians who wish to request limits to the sharing of their child’s data stored in ALERT.
