Immunization information systems (IIS) are confidential, population-based, computerized databases that record all immunization doses administered by participating providers to persons residing within a given geopolitical area. This information details how to access immunization records for vaccine recipients who received a vaccine in Oklahoma.
General Information
Oklahoma's IIS is the Oklahoma State Immunization Information System (OSIIS), and it includes immunization records for vaccine recipients of all ages. Visit this webpage to learn more about the IIS.
Child Consent to Participate
For children, OSIIS requires implicit consent with opt-out by operational policy. Children are included in the IIS unless their parent(s)/guardian(s) choose to opt them out.
Adult Consent to Participate
For adults, OSIIS requires implicit consent with opt-out by operational policy. Adults are included in the IIS unless they choose to opt out.
Provider Reporting
Oklahoma does have a mandate for vaccination providers to report immunizations to OSIIS. The operational reporting mandate:
- Applies only to specific immunizations (i.e., COVID-19 vaccines, VFC Program vaccines).
