Immunization information systems (IIS) are confidential, population-based, computerized databases that record all immunization doses administered by participating providers to persons residing within a given geopolitical area. General Information

Child Consent to Participate

For children, OSIIS requires implicit consent with opt-out by operational policy. Children are included in the IIS unless their parent(s)/guardian(s) choose to opt them out.

Adult Consent to Participate

For adults, OSIIS requires implicit consent with opt-out by operational policy. Adults are included in the IIS unless they choose to opt out.

Provider Reporting

Oklahoma does have a mandate for vaccination providers to report immunizations to OSIIS. The operational reporting mandate:
