What to know

Immunization information systems (IIS) are confidential, population-based, computerized databases that record all immunization doses administered by participating providers to persons residing within a given geopolitical area. This information details how to access immunization records for vaccine recipients who received a vaccine in Ohio.

General Information

Ohio's IIS is the Ohio Impact Statewide Immunization Information System (ImpactSIIS), and it includes immunization records for vaccine recipients of all ages. Visit this webpage to learn more about the IIS.

The jurisdiction is authorized by law to operate ImpactSIIS. For more information, please see Ohio Revised Code 3701.13(B)(4).

Child Consent to Participate

For children, ImpactSIIS requires implicit consent with opt-out by law. Children are included in the IIS unless their parent(s)/guardian(s) choose to opt them out.

For adults, ImpactSIIS requires implicit consent with opt-out by law. Adults are included in the IIS unless they choose to opt out.

Provider Reporting

Ohio does not have a mandate for vaccination providers to report immunizations to ImpactSIIS. The operational reporting policy:

  • Establishes voluntary reporting.

Data Sharing

Ohio does have a policy that regulates sharing data from ImpactSIIS with other public health entities. The data sharing policy:

  • Requires a data use/sharing agreement or memorandum of understanding,
  • Requires that the data are only used for public health or other specific purposes,
  • Allows data to be shared upon request or with approval/authorization, and
  • Allows only deidentified data or nonidentifying summary statistics to be shared.
