Immunization information systems (IIS) are confidential, population-based, computerized databases that record all immunization doses administered by participating providers to persons residing within a given geopolitical area. This information details how to access immunization records for vaccine recipients who received a vaccine in North Carolina.

General Information

North Carolina's IIS is the North Carolina Immunization Registry (NCIR), and it includes immunization records for vaccine recipients of all ages. Visit this webpage to learn more about the IIS.

Child Consent to Participate

For children, NCIR requires mandatory inclusion by law. Children are included in the IIS with no possibility for their parent(s)/guardian(s) to opt them out.

Adult Consent to Participate

For adults, NCIR requires mandatory inclusion by law. Adults are included in the IIS with no possibility to opt out.

Provider Reporting

North Carolina does have a mandate for vaccination providers to report immunizations to NCIR. The reporting mandate:
