What to know

Immunization information systems (IIS) are confidential, population-based, computerized databases that record all immunization doses administered by participating providers to persons residing within a given geopolitical area. This information details how to access immunization records for vaccine recipients who received a vaccine in New York State.

General Information

New York State's IIS is the New York State Immunization Information System (NYSIIS), and it includes immunization records for vaccine recipients of all ages. Visit this webpage to learn more about the IIS.

The jurisdiction is authorized by law to operate NYSIIS. For more information, please see Consolidated Laws of New York Chapter 45, Article 21, Title 6 § 2168(1).

Child Consent to Participate

NYSIIS requires mandatory inclusion for child vaccine recipients ages 0 to 18 years by law. Children are included in the IIS with no possibility for their parent(s)/guardian(s) to opt them out.

The child consent policy is based on alignment with the jurisdiction's provider reporting mandate.

Adult Consent to Participate

For adults who are 19 years of age or older, NYSIIS requires explicit consent with opt-in by law. These adults must give consent, or opt in, to participate in the IIS.

Provider Reporting

New York State does have a mandate for vaccination providers to report immunizations to NYSIIS. The reporting mandate:

  • Applies only to a subset of vaccine recipients and
  • Applies only to certain providers.

Data Sharing

New York State does have a policy that regulates sharing data from NYSIIS with other public health entities. The data sharing policy:

  • Requires that the data are only used for public health or other specific purposes,
  • Allows data to be shared upon request or with approval/authorization,
  • Allows only deidentified data or nonidentifying summary statistics to be shared.
