What to know

Immunization information systems (IIS) are confidential, population-based, computerized databases that record all immunization doses administered by participating providers to persons residing within a given geopolitical area. This information details how to access immunization records for vaccine recipients who received a vaccine in New York City.

General Information

New York City's IIS is the Citywide Immunization Registry (CIR), and it includes immunization records for vaccine recipients of all ages. Visit this webpage to learn more about the IIS.

The jurisdiction is authorized by law to operate CIR. For more information, please see New York City Health Code § 11.07(b).

Child Consent to Participate

For children, CIR requires mandatory inclusion by law. Children are included in the IIS with no possibility for their parent(s)/guardian(s) to opt them out.

Adult Consent to Participate

For adults who are 18 years of age, CIR requires mandatory inclusion by law. These adults are included in the IIS with no possibility to opt out.

For adults who are 19 years of age or older, CIR requires explicit consent with opt-in by law. These adults must give consent, or opt in, to participate in the IIS.

Provider Reporting

New York City does have a mandate for vaccination providers to report immunizations to CIR. The reporting mandate:

  • Applies only to a subset of vaccine recipients and
  • Applies only to certain providers.

Data Sharing

New York City does have a policy that regulates sharing data from CIR with other public health entities. The data sharing policy:

  • Requires that the data are only used for public health or other specific purposes,
  • Allows data to be shared upon request or with approval or authorization, and
  • Allows only deidentified data or nonidentifying summary statistics to be shared.
