Immunization information systems (IIS) are confidential, population-based, computerized databases that record all immunization doses administered by participating providers to persons residing within a given geopolitical area. Child Consent to Participate

For children, NHIIS requires explicit consent with opt-in by law. Parent(s)/guardian(s) must give explicit consent, or opt in, for their children to participate in the IIS.

Provider Reporting

New Hampshire does not have a mandate for vaccination providers to report immunizations to NHIIS. The reporting policy:
