Immunization information systems (IIS) are confidential, population-based, computerized databases that record all immunization doses administered by participating providers to persons residing within a given geopolitical area. This information details how to access immunization records for vaccine recipients who received a vaccine in Kentucky.

Kentucky’s IIS is the Kentucky Immunization Registry (KYIR), and it includes immunization records for vaccine recipients of all ages. Visit this webpage to learn more about the IIS.

The jurisdiction is authorized by law to operate KYIR. For more information, please see Kentucky Revised Statutes 211.180.

Child Consent to Participate

For children, KYIR requires implicit consent with opt-out by operational policy. Children are included in the IIS unless their parent(s)/guardian(s) choose to opt them out.

Adult Consent to Participate

For adults, KYIR requires implicit consent with opt-out by operational policy. Adults are included in the IIS unless they choose to opt out.

Provider Reporting

Kentucky does not have a mandate for vaccination providers to report immunizations to KYIR. The reporting policy:

  • Establishes voluntary provider reporting.

Data Sharing

Kentucky does have a policy that regulates sharing data from KYIR with other public health entities. The data sharing policy:

  • Requires that the data are only used for public health or other specific purposes and
  • Requires protecting the data’s confidentiality
