Immunization information systems (IIS) are confidential, population-based, computerized databases that record all immunization doses administered by participating providers to persons residing within a given geopolitical area. General Information

Hawaii's IIS is the Hawaii Immunization Registry (HIR), and it includes immunization records for vaccine recipients of all ages. Visit this webpage to learn more about the IIS.

Child Consent to Participate

For children, HIR requires implicit consent with opt-out by law. Children are included in the IIS unless their parent(s)/guardian(s) choose to opt them out.

Provider Reporting

Hawaii does not have a mandate for vaccination providers to report immunizations to HIR. The reporting policy:
