COVID-19 Emergency Use Authorization Recipient and Caregiver Fact Sheet and VIS Codes
Vaccine Information Statements (VIS), are created for FDA BLA licensed vaccines. CDC authored VIS will not be available for COVID-19 vaccines while they are under Emergency Use Authorization (EUA). For vaccines under an EUA, the FDA requires that a manufacturer/vaccine specific EUA Recipient/Caregiver Fact Sheet be provided to the vaccine recipient or their caregiver.
