  • Funding also supports improving health outcomes for people diagnosed with diabetes.

Innovative State and Local Public Health Strategies to Prevent and Manage Diabetes, Heart Disease, and Stroke

This 5-year cooperative agreement began in October 2018. It funded 21 state health departments, large city and county health departments, and a consortium of city and county health departments.

These health departments worked on developing innovative approaches to increase the reach and effectiveness of evidence-based public health strategies. They sought to do so for populations and communities with a high burden of diabetes, heart disease, and stroke.
