This short form allows community health workers to gather feedback from participants about the AMIGAS education sessions. The information collected will help community health workers improve their sessions and make them clear, understandable, and enjoyable for the participants.

How to use the evaluation form

The form can be offered on paper for in-person sessions, or as an online survey. The form or survey is given at the end of both individual and group sessions as described in the lesson plans. Community health workers can help the women complete the form or survey and answer any questions they might have.
