How to use the appointment card

Community health workers can give this card to a woman to use when she is ready to make an appointment. The woman can write the date, time, and location of her appointment on the card as a reminder to herself.

  1. Download and save the appointment card file.
  2. If desired, customize the appointment cards.
  3. Print the file double-sided and clip the cards.
