Summary

Neurologists within Texas were contacted by the Texas Department of State Health Services and asked to complete a one-page case reporting form for each of their patients treated between 2009 and 2011. The project is a continuation of the previously published Texas-specific epidemiological publication; findings were consistent among projects.
