An Innovative Approach to Using Electronic Health Records Through Health Information Exchange to Build a Chronic Disease Registry in Michigan

A diagram describes the phases of CHRONICLE’s planning and implementation, from 2019 through 2023. From 2019 through 2021, the objectives of an EHR-based chronic disease registry were established, the potential and completeness of ADT for chronic disease surveillance was examined in coordination with HIE, and the models and storage solutions that could support longitudinal chronic disease data collection were explored. In 2022, the key system requirements and disease monitoring workflow were outlined and prioritized, partner pilot health systems were identified, data sharing agreements were established, and an initial build of CHRONICLE was deployed to begin data collection, evaluation, and testing. In 2023, the data collected in CHRONICLE were analyzed to establish baseline measurements, document data elements and their value sets, and identify areas for improvement. Additionally, the system was tested and refined to streamline data processing and improve usability, and an administrative rule with the goal of enabling chronic disease reporting statewide was drafted. The next CHRONICLE implementation phase in 2024 will focus on system expansion and data validation.

The dashboard page displays an example of a CHRONICLE report, which includes the trends in visits for stroke and hypertension and bar charts for encounters by patient sex and age categories. The left-hand side of the dashboard page features a selection of filters that can be applied to the trend graph and charts.
