Partners Incorporate Vaccines Into Community Care

Success Story

Public Health Institute, funded under the IP21-2113 cooperative agreement, worked with RISE-North Carolina, Shackle Free, Action NC, and Conetoe Family Life and Social Change in Warsaw, North Carolina to launch a multifaceted health initiative to improve community health outcomes. They set up onsite office hours at pharmacies and can track vaccination participation among referred community members. Teaming up with Conetoe Family Farm, they’ve gone beyond vaccines, offering a range of essential resources, including fresh produce, food, diapers, toys, and gift cards, thereby addressing broader community needs.

Using their Community Health Ambassador Call Center and the DialMyCalls app, RISE and Shackle Free proactively engaged with residents, sharing vital information about COVID-19 vaccine programs. This outreach has resulted in connecting 232 individuals to COVID-19 vaccines.

In addition to administering 2,605 flu vaccines, they have provided 175 pneumonia, 301 shingles, 71 RSV, and 161 other vaccines, including HPV through their monthly clinics. They also partnered with Higher Ground Transportation to take individuals with transportation barriers to the nearest provider.

Through their dedication and innovative initiatives, RISE is not only administering vaccines but also fostering a healthier, more resilient community in Warsaw and beyond.
