Project IMPACT (Implementing Million Hearts for Provider and Community Transformation)

Description

In the U.S., nearly one-third of adults have high blood pressure, Type 2 diabetes has grown to epidemic proportions, and heart disease is the leading cause of death.  The burden of high blood pressure, diabetes and premature death related to heart disease affects a greater portion of racial ethnic minority populations. Immigrant racial and ethnic groups such as Asian and Hispanic American adults have a relatively unique profile of heart disease risk factors, including higher rates of high blood pressure and diabetes overall, and at normal weight, low rates of high cholesterol. Addressing different language and cultural barriers is important in preventing and managing heart disease among different racial and ethnic minority groups because these barriers are main reasons why individuals do not seek or receive adequate health care.

The NYU-CUNY Prevention Research Center (PRC) will build on the U.S. Department of Health and Human Services’ Million Hearts Initiative.   The core project, Project IMPACT (Implementing Million Hearts for Provider and Community Transformation), will research and integrate a community health worker model with physician support models using electronic health record tools to improve blood pressure control among South Asians in New York City.   To conduct this research, the NYU-CUNY PRC will collaborate with health insurance organizations and their provider networks in NYC.

Project IMPACT will help public health practitioners, community partners, academic researchers, and students of public health and medicine better understand how to reduce heart disease and health disparities in immigrant communities through research, community engagement, and training.
