A booming medspa industry is quietly operating in a regulatory gray zone, where vitamins and supplements used in IV infusions face little oversight, and accountability becomes murky when patients are harmed.

In September 2025, Santa Clara County Public Health investigated two ICU admissions due to septic shock after IV infusions at a medical spa, only to encounter limited cooperation and unclear medical supervision. A surprise site visit revealed a nurse-run facility tied to an international franchise and sourcing supplements through informal cross-border channels with virtually no traceable quality controls.

The case exposed significant gaps in legislation around supplement sourcing, infection control, and enforcement standards for medspas in California, and around the country. Ultimately, this incident underscores how a single adverse event can illuminate systemic failures in accountability within a rapidly expanding healthcare-adjacent industry.
