Background: The Pennsylvania Department of Health was funded as a standard program in June 2008.

Lifestyle Intervention:  Pennsylvania will use core elements from A New Leaf...Choices for Healthy Living to help women develop a healthier diet, increase physical activity, and be tobacco free. Core elements include risk assessments of diet, physical activity, and smoking; tailored feedback as cues to action; goal-setting and action planning; guidelines and strategies that help participants overcome barriers to healthy eating, increasing physical activity, and smoking cessation; follow-up and reinforcement to assess progress in reaching, and opportunities for setting new, goals; and social support encouragement for sustained lifestyle change.

Screening: Risk factor screening includes blood pressure, glucose, cholesterol and other lipids, as well as assessment of weight, body mass index, personal and family medical history, tobacco use, diet, and physical activity.

Sites: Health care provider sites under the umbrella of The Alliance of Pennsylvania Councils, beginning with three health care centers in medically underserved areas of Philadelphia, maintained by the Philadelphia Department of Public Health (PDPH). PDPH is a Breast and Cervical Cancer Early Detection Program (BCCEDP) provider, and serves as a "safety net" provider of medical services in Philadelphia, providing primary care and low-cost or free prescriptions to residents, regardless of their ability to pay.

Key Partner: The Department's BCCEDP (HealthyWoman Program), American Heart Association, National Nursing Centers Consortium, and Drexel University.
