This is a listing of visa requirements for official U.S. passports for entry into foreign countries. The listing shows the number of visa application forms with original signature and photos required to apply for the Visa as well as any other requirements (i.e. letter of invitation, OMEGA printed itinerary, etc.). Please contact the CDC Passport/Visa Services Office (404-498-2952 or email CDCPASS@cdc.gov) for assistance in completing your application package. The Request for Passport/Visa Services (CDC form 0.1308) is required for all passport and visa requests.

It is important to note that entry requirements for U.S. citizens traveling on official U.S. passports often vary from entry requirements for U.S. tourists. Entry requirements for foreign passport holders also vary and entry requirements must be verified with COGH on a trip-by-trip basis.

NOTE: It is the traveler's responsibility to obtain letters of invitation, visa receipt numbers (Russia) and visa authorization issuance numbers (Vietnam).

Many visa applications are available online in Adobe Acrobat Reader (PDF) format. You will need the Adobe Acrobat Reader on your computer to open a form. Click on a country name below to open a form or right-click and select "Save Target As..." to save a copy on your computer. Most of these forms have been designed to allow you to fill them in online. You can then print or save a copy to your computer.

Iran			No travel allowed. No diplomatic relations with U.S. -- do not put an Iranian visa in an official U.S. passport.
Iraq			No diplomatic relations with U.S. -- do not put an Iraqi visa in an official U.S. passport. U.S. passports are not good for travel to Iraq unless validated by State Department.
Ireland (UK)	0	0	No visa required up to 6 months.
