The forms for reporting cases of
viral hepatitis to CDC are currently being revised. The new forms
will be used to report cases of viral hepatitis through the new
National Electronic Disease Surveillance (NEDSS). Drafts of these
forms are provided here although use of these forms on a nationwide
basis will only occur once NEDSS is operational.
